Declaration of marriage or civil union

On the day of the solemnization, you must complete the Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55) form.

These forms are available in English and French, only in paper versions.

Each form is numbered and has four pages, in the following order:

  • The original Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55), page 1 – white sheet;
  • The original Return of Marriage (SP−2) or Return of Civil Union (SP−7), page 2 – white sheet;
  • The spouses’ copy of the Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55), page 3 – green sheet;
  • The Officiant’s copy of the Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55), page 4 – yellow sheet.

Signature of the declaration and return of marriage

At the end of the ceremony, the officiant, the spouses and the two witnesses must sign the Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55) completed earlier. The officiant must give the spouses their copy of the form (green copy).

The officiant and the spouses must also sign the Return of Marriage (SP−2) or Return of Civil Union (SP−7) on the second page of the declaration.

How to obtain a declaration form

The designated officiant does not take any steps to obtain the Declaration of Marriage (DEC−50) or Declaration of Civil Union (DEC−55) form. The document is mailed to him along with the authorization letter.

For the other categories of officiants, the Declaration of Marriage (DEC−50) and Declaration of Civil Union (DEC−55) forms are provided by the Directeur de l’état civil on presentation of the officiant number. The request may be made by telephone.
