Departments and agencies that will be notified or that can be notified of the death

Agreements with various departments and agencies authorize the Directeur de l’état civil to provide them with the information necessary for the application of the laws and programs they administer. The agreements set forth operating procedures in compliance with the applicable laws, particularly regarding the protection of personal information. Only authorized personnel in the departments and agencies concerned will have access to this information.

Below are the departments and agencies that will automatically be notified of the death, and those that can be, according to the answers provided by the declarant of the death on the form Application for the simplified forwarding of information relative to the death.

Régie de l’assurance maladie du Québec To cancel registration in the Health Insurance Plan and, if applicable, in the Public Prescription Drug Insurance Plan. You will have no other formality to complete with regard to these cancellations. To contact the Régie de l’assurance maladie du Québec.

Retraite Québec - Québec Pension Plan and the Family Allowance For the administration of the Québec Pension Plan and the Family Allowance. You will still have to contact Retraite Québec to find out what other formalities need to be completed to apply for a benefit, pension or other indemnity paid to a surviving relative. Retraite Québec - Public sector pension plans (RREGOP, PPMP or other) To ensure that no proof of death is required of the deceased's relatives as part of the formalities they must complete to benefit from the deceased's public sector pension plan. You will still have to contact Retraite Québec to find out what other formalities need to be completed to apply for benefits tied to the deceased's pension plan. To contact Retraite Québec.

Ministère du Travail, de l'Emploi et de la Solidarité sociale For the administration of the Programme d’aide sociale and the Programme de solidarité sociale (including the recovery of amounts owed). You will still have to contact the Ministère in some situations. Revenu Québec For the administration and application of the laws and programs for which it is responsible. You need to contact Revenu Québec to know the other formalities relating to the death that need to be completed. Canada Revenue Agency For the administration of the Income Tax Act. You will still have to contact the CRA in some situations. Service Canada (under the responsibility of Employment and Social Development Canada) To cancel registration in the Social Insurance Register (social insurance number). You will have no other formalities to complete with regard to this registration. To contact Service Canada.

Curateur public du Québec To ensure that no proof of death is required from the relatives of the deceased who was under protective supervision, or the legal representative of an incapacitated person, as part of the formalities they must complete with the Curateur public du Québec. You will still have to contact the Curateur public for information on the other formalities to be completed in connection with the death. To contact the Curateur public du Québec.

Ministère du Travail, de l’Emploi et de la Solidarité sociale – Québec Parental Insurance Plan To ensure that the file of a deceased person who submitted an application for benefits under the Québec Parental Insurance Plan or received such benefits is processed. You will still have to contact the Ministère for information on the other formalities to be completed in connection with the death. Régie du bâtiment du Québec To ensure that the public register of licence holders kept by the Régie du bâtiment du Québec is updated, where the deceased held a licence issued by the Régie du bâtiment. Indigenous Services Canada To ensure that the Indian Register is updated, where the deceased had Indian status under the Indian Act. Ministère de la Santé et des Services sociaux - Direction des affaires autochtones To have the Register of Cree and Naskapi Beneficiaries updated and so that proof of death will not be required from the deceased's relatives as part of the formalities to be completed in connection with the death. The local registry officer will contact you to inform you of the other formalities to be completed in connection with the death. Chief Firearms Officer of Québec To have the file of the deceased updated and so that proof of death will not be required from the deceased's relatives as part of the formalities to be completed in connection with the death. If the deceased possessed one or more firearms, the liquidator of the succession must still contact the RCMP's Canadian Firearms Program for more information on the other formalities to be completed concerning the disposal of the firearm(s). Société de l'assurance automobile du Québec (SAAQ) To have the file of the deceased updated and so that proof of death will not be required from the deceased's relatives as part of the formalities to be completed in connection with the death. You must still contact the SAAQ for information on the other formalities to be completed concerning the reimbursement of costs related to the driver's licence or its cancellation, to the storage of a vehicle or the transfer of its ownership. Commission des normes, de l'équité, de la santé et de la sécurité du travail To have the file of the deceased updated and so that proof of death will not be required from the deceased's relatives as part of the formalities to be completed in connection with the death. You must still contact the Commission for information on the other formalities to be completed in order to receive any indemnities. Ministère de la Sécurité publique - Service d'immatriculation des armes à feu du Québec To have the file of the deceased updated and so that proof of death will not be required from the deceased's relatives as part of the formalities to be completed in connection with the death. If the deceased possessed one or more firearms, the liquidator of the succession must still contact the Service d'immatriculation des armes à feu for more information on the other formalities to be completed concerning the transfer of ownership of the firearm(s).
