Requests to correct information in a certificate or a copy of an act

A person who believes there is an error in an act for an event entered in the register of civil status may ask the registrar of civil status to correct it. Corrections may be requested for any information contained in the register of civil status. Check who may request a correction depending on the event:

Who may request a correction?
Type of event Who may submit the request? Who must consent?
Birth Person of full age concerned by the error;
One of the parents of the minor child concerned by the error;
One of the parents of the person of full age concerned by the error.
Person of full age concerned by the error;
Minor child: both parents of the person concerned
and the minor child aged 14 or over;
The parent of the minor child, if the error concerns the parent’s name.
Marriage Either spouse. Both spouses.
Civil union Either civil union spouse. Both civil union spouses.
Death The declarant;
The funeral director or their authorized representative;
The deceased’s spouse;
The deceased’s civil union spouse ;
The deceased’s mother or father;
The deceased’s children.
If you are not the person who declared the death
and you are among those authorized to submit the
request, you must check the box in the form confirming
your commitment to inform the persons concerned by the succession.

You must send us the documents concerned without delay.

Important: Do not enter the corrections you want made directly on the documents.

You must fill out the Request to correct a life event in the register of civil status form to request a correction of a life event and send it by mail to the Directeur de l’état civil. Your form as well as the original documents issued by the Directeur de l’état civil covered by the request, if applicable, must be sent by mail to the following address:

We will review the information contained in the document. If we see that there is an error, we will make the necessary correction within the limits of the powers conferred on us by law. Otherwise, the document will be returned to you.
