Application for a certificate or copy of an act

To use the service

You must complete the appropriate application form, depending on whether you want a certificate or a copy of an act. There is a form for each of the following events: birth, marriage or civil union and death.

When to use the service

This service is used to submit an application for a certificate or copy of an act of birth, marriage, civil union or death by mail.

Who is the service for?

This service is for you if

  • you do not have access to the Internet
  • you are unable to obtain an identifier (user code and password) for clicSÉQUR, the government authentification service

Methods of payment accepted

Visa or MasterCard master cards*, cheque, postal money order, bank money order
* Visa and MasterCard debit cards are not accepted.

Terms of use

You must

  • be mentioned in the act that pertains to the document requested, or provide a document establishing your interest in filing an application
  • establish your identity by attaching to your application two photocopies of documents issued by two separate organizations:
    • a photocopy of a valid photo ID
    • a photocopy of a valid proof of home address

Information

We offer normal or accelerated processing of applications.

In the case of a recent event, the time period for entering an event in the register must be taken into account. We cannot process your application for a certificate or copy of an act until the event has been registered.


Residents outside Canada

If you choose to send your application through a private shipping service, please note that you (the sender) are responsible for all of the associated fees for sending it, including transportation, duties and taxes as well as any other related costs.

To avoid any problem related to sending your document, please pay close attention to the required formalities. For more information, we recommend that you contact the shipping service.
